The Choice is Yours
We will provide you with information to help you make informed choices, such as physicians' and health care professionals' credentials. This information, however, is not an endorsement of a particular physician or health care professional's suitability for your needs.

The providers available through this application do not necessarily reflect the full extent of Oxford’s network of contracted providers. There may be providers or certain specialties that are not included in this application that are part of our network. If you don't find the provider you are searching for, you may contact the provider directly to verify participation status with Oxford’s network, or contact Customer Service at the toll-free number shown on your Member’s ID card.


If your Member ID card says that you are covered under the Liberty PlanSM, and you choose to see a doctor who participates in the Freedom® network rather than the LibertySM network, all care provided by that doctor will be considered out-of-network care.  Depending on the terms of your Certificate of Coverage, you may be responsible for most or all of the charges incurred during your visit(s).

The AARP MedicareComplete Mosaic plan provider network is not searchable through this tool at this time. For a listing of participating providers please select the 'Mosaic Provider Directory' link located at the bottom of the Provider Search Tool.

Search for a Physician

  1. Select one of the following search options:
    1.   You can search for a provider by entering a location you wish to search from.  You can also narrow your search by entering or selecting optional criteria.
      • Step 1.  Select the number of miles to search for a physician based within a location you will enter in Step 2 (default value is 5 miles).
      • Step 2.  Enter the required fields City & State OR ZIP Codemandatory
      • Step 3.  Select a Physician Type (e.g., PCP/OBGYN, Specialist or Dentist).  mandatory
      • Step 4.  In order to narrow your search, you can enter optional search criteria (e.g., Street Address, Specialty, Network, Gender, Language or Participating Hospital Affiliations).
      OR
    2.   You can search for a provider by entering all or part of his/her last name, along with a location you wish to search from.  You can also narrow your search by entering or selecting optional criteria.
      • Step 1.  Enter at least the first 2 letters of the physician’s Last Namemandatory
      • Step 2.  Select the number of miles to search for a physician based within a location you will enter in Step 3 (default value is 5 miles).
      • Step 3.  Enter the required fields City & State OR ZIP Codemandatory
      • Step 4.  Select a Physician Type (e.g., PCP/OBGYN, Specialist or Dentist).  mandatory
      • Step 5.  In order to narrow your search, you can enter optional search criteria (e.g., at least 1 character of the physician's First Name, Street Address, Specialty, Network, Gender, Language or Participating Hospital Affiliations).
      OR
    3.   You can search for a specific provider by entering a valid Oxford Provider ID.
      • Step 1.  Enter an Oxford Provider IDmandatory
  2. Click on the Search button.
  3. View the list of providers.  You can view your results by using various options listed below.  If your search returns no results, you can change your search criteria by clicking on the New Search button for a completely new search or the Refine Search button to adjust your current search.
    1. Navigate through the provider results based on the current sort by clicking on the navigation links at the top and bottom of the results:
      • First Page – Click on this link to display the first page.
      • Previous Page – Click on this link to display the previous page in numerical order.
      • Next Page – Click on this link to display the next page in numerical order.
      • Last Page – Click on this link to display the last page.
      • Page number 1 | 2 | 3 | 4 | 5 – Click on any of these links to bring you to a specified page.
    2. Sort your results differently if you used the By Location or By Name search.  Your results are automatically sorted by miles.  If you wish to sort your results differently, click on the headings that contain arrows (Miles and Contact Information).  The sorted heading will be highlighted.  Clicking on the same heading will sort the results between ascending and descending.
    3. Print or save your results.  You can print or save all or part of the results returned by using one of the following options:
      • Print This Page:  Click on the Print This Page button to print only those providers who are listed on the page in view.
      • Save Results:  Click on the Save Results button to save and/or print all of the provider results as a PDF file.  If you do not have Adobe Reader installed on your computer, you can download the software by clicking on the Download Adobe Reader link located at the bottom of the page.
    4. View more details for a provider.  If you wish to view a specific provider or hospital in more detail, click on the provider’s name link from the results page.  The detail page for that provider will display.  Click on the back button if you wish to return to the results page.

    Name/Address/Phone
    Except as otherwise noted, all information about this physician’s office location, languages, hospital affiliation, office hours and acceptance of new patients is self reported by the physician and only updated upon his or her request.


Physician Type Definitions

Specialty is self-reported by the physician and verified when the physician first joins the network by verification of the physician’s training in the specialty or his/her Board Certification status.

Primary Care Physician (PCP)/OBGYN (Obstetrics and Gynecology)
A participating physician whose area of practice and training is Family Practice, General Medicine, Internal Medicine, Pediatrics or Obstetrics and Gynecology.  A PCP has agreed to provide primary care services and to coordinate and manage all covered services for Members who have selected such a participating physician.  See: Primary Care Physicians/OBGYN Specialties.
Specialist
A participating physician who provides medical care in any generally accepted medical or surgical specialty or subspecialty.  See: Specialist Specialties.
Dentist
A participating, licensed practitioner who is skilled in the prevention, diagnosis and treatment of diseases, injuries and malformations of the teeth, jaws and mouth, and who inserts false teeth.  See: Dentist Specialties.

Search for a Hospital/Ancillary Facility Instructions

*Hospital Accreditation status is verified directly with The Joint Commission, www.jointcommission.org or American Osteopathic Association (AOA) www.osteopathic.org when the provider is first credentialed and then at least every three years thereafter.     2.   You can search for a hospital or facility by entering all or part of the facility name, along with a location you wish to search from.  You can also narrow your search by entering or selecting optional criteria.
      • Step 1.  Enter at least 3 letters of the Facility Namemandatory
      • Step 2.  Select the number of miles to search for a facility based within a location you will enter in Step 3 (default value is 5 miles).
      • Step 3.  Enter the required fields City & State OR ZIP Codemandatory
      • Step 4.  In order to narrow your search, you can enter optional search criteria (e.g., Street Address, Facility Type, Specialty or Network)
      OR
    3.   You can search for a specific facility by entering a valid Oxford Provider ID.

  2. Click on the Search button.

Facility Type Definitions

Hospital/Ancillary Facility
A participating institution where the sick or injured are given medical or surgical care.  See: Hospital Specialties or Ancillary Facility Specialties.

Network

Participating physicians, hospitals and other healthcare providers who contract with UnitedHealthcare/Oxford to participate in health benefits plans.  A Member must access care through the Member's plan-specific network to receive the maximum level of benefits.

The AARP MedicareComplete Mosaic plan provider network is not searchable through this tool at this time. For a listing of participating providers please select the 'Mosaic Provider Directory' link located at the bottom of the Provider Search Tool.

Alternative Medicine provider services are subject to the terms of the Member’s Certificate of Coverage.


Only those providers who participate in the plan listed on your Oxford Member ID card will be considered in-network providers.  If your Member ID card says that you are covered under the Liberty PlanSM, and you choose to see a provider who participates in the Freedom® network rather than the LibertySM network, all care provided by that provider will be considered out-of-network care.  Depending on the terms of your Certificate of Coverage, you may be responsible for most or all of the charges incurred during your visit(s).


Board Status

Board certification is voluntary. A Board certified physician, after completing residency training in their specialty, has passed an exam and met all the requirements established by their Board. This information is verified with the American Board of Medical Specialties or the American Osteopathic Association when the physician first joins the network and at least every three years thereafter. You can verify the physician´s current Board status by visiting www.abms.org.

Board-Certified
A board-certified physician has been certified by a Member Board of the American Board of Medical Specialties (ABMS) and has successfully completed an approved educational program and an evaluation process, which includes an examination designed to assess the knowledge, skills and experience required to provide quality patient care in a particular specialty.
Board-Eligible
Providers are considered board-eligible when at least five years have passed since they completed their residency or fellowship.  These providers are typically in the process of obtaining their certification.

Participating Hospital Affiliations

Individual physicians may be affiliated with one or more hospitals.  You can limit your search to physicians affiliated with a specific hospital by entering all or part of the hospital name in the Participating Hospital Affiliation field.


Limit Results to the County?

If you wish to search for providers located in a specific county, you can limit your search by selecting the Yes button in the Limit Results to the County? field. The results will return all providers within the same county as the City & State OR ZIP Code you entered.

This can help you find a nearby provider within the same county, even when you are close to a state or county border.

*Note: If you are searching for providers within an entire county, you should increase the Within miles up to 100.
